CHIP application and renewal assistance

To get Medicaid or CHIP your child must be:

• Age 18 or younger (i some cases children with disabilities age 19 or 20 can get Medicaid)
• A Texas resident and a U.S. citizen or qualified non-citizen
• You must bring the following documents to your appointment:
  • Government issued ID
  • Birth certificates
  • Social Security numbers
  • Job and income information, such as paystubs
  • Proof of address
