Center for Transforming Lives is currently seeking a full-time Case Manager for the Transitional Housing Program to work Monday-Friday and occasional Saturday hours as needed. The Case Manager is responsible for providing trauma informed care to aid in the successful transition for both the residential and home based case management services. The Case Manager will also be responsible for developing relationships with local landlords, coordinating services with local shelters, screening and assessing participants for the program, and ensuring compliance with program expectations. In addition, the Case Manager will be responsible for assisting participants with obtaining the necessary resources to achieve service plan goals. The Case Manager position does entail travel for building relationships with community partners and providing home based services to participants.
Qualified candidates have a minimum of a Bachelor’s degree in Social Work, Psychology, Sociology, or related field is required. A Master’s degree is preferred.
Two years of experience working with women and families in crisis, people who are currently or were previously homeless;
Knowledge and the application of trauma informed care;
Working with individuals living with various mental health diagnoses and/or recovering from substance/alcohol abuse;
Experience working with individuals living in emergency shelter/transitional housing;
Experience applying a clinical approach to evaluating and documenting services;
Experience working in the field of social services, with a wide range of knowledge about the availability of community resources is required.
Knowledge of Windows operating system and Office products, including Microsoft Word, Excel and Outlook. Ability to learn and use any other software programs necessary for the office.
